What personal data we collect and why we collect it
Levrx collects personally identifiable information within the application to verify and authenticate users, facilitate app features, and to perform the communication services requested by users within the application. Additionally, Levrx collects information about how you use the application in order to gauge user interest in application features and to improve the application.
Contact Information
Contact information, including your Name, Email Address, and Phone Number are collected and linked to you user to verify and authenticate you as a user, to personalize your user experience, and when submitting requests on your behalf and at your instruction to pharmacies and doctor’s offices.
Location
Your Precise Location is used, but not collected or stored, to assist you in in searching for nearby pharmacies when you use the pharmacy search feature.
User Content
Your Medication Information, Pharmacy Information, Medical Provider Information, and your Notes are collected when you freely enter and save this information within the application for your own future use. This information is also used when submitting requests on your behalf and at your instruction to pharmacies and doctor’s offices.
Search History
Your Medications Search History and Pharmacy Search History is collected for analysis purposes to improve the application and enhance user experience by predicting search values. This information is de-identified and is not linked to your user account.
Identifiers
Your User ID is collected and stored in order to identify you and authenticate you on login.
Diagnostics
Application Crash Data is collected for analysis purposes to improve the application. This information is de-identified and is not linked to your user account.
Cookies
Cookies are used to determine if you have already registered and confirmed a user when you open the application on your device, and for your security, in order to determine how long it has been since your User ID on this device was secured using a second factor.
Who we share your data with
We don’t share your personal information with third parties. Only aggregated, anonymized data is periodically shared with organizations like your health insurance company to help us improve the Levrx app and demonstrate how the app is working to help users reduce prescription costs and improve care management. None of this information is shared with or sold to advertisers.
How long we retain your data
For users that register in the app, we store the personal information in your user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Our support and administrative staff can also see and edit some of your information. Because the app involves healthcare data “personal health information” all data is backed up and stored per state and federal requirements, for at least ten years.
What rights you have over your data
You can request that we erase any personal data, if any exists, we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes, like personal health information. You may also request copies of your data at any time by contacting appsupport@levrx.com.
